Appearance Release Forms

By policy, it is required that producers acquire the written consent of persons appearing on their program. This is to protect your program as well as the station. When on location, please acquire the consent of anyone whom you interview or videotape.

*TIP* If you can't get this printed out, then just record them giving you permission to air their interview.

Program Proposal

Are you interested in submitting a program to our station? If you are bringing a new program, please fill out a program proposal, with the required fields: YOUR NAME, ADDRESS, PHONE NUMBER, ORGANIZATION, PROGRAM NAME, LENGTH, and DESCRIPTION.

If you are an out-of-state producer interested in having your program aired in Hartford, CT: Residency in Hartford is required, or at least a sponsor who lives in Hartford. Please contact us by phone (860) 524-8621 for further information before submitting tapes with this proposal.

All producers must fill out one of these forms when they drop off a tape. These aid us in accurately labeling your tapes and programs. To facilitate your visit to our station, please feel free to download and fill out the information on the slip before coming down.

*REMINDER* Each program must be accompanied by a Drop Off Slip. If you are delivering multiple programs, fill out one slip per tape, and make certain to attach each slip to the proper tape. (This way, the episode meant to air one week, will not get mixed up with the episode for the following week!)
